IMMUNIZATIONS
Immunization requirements for children to attend preschool and schools in New Jersey, both public and private, are established by the New Jersey Department of Health NJDOH.
Public schools in New Jersey are required by N.J.A.C. 6A:16- 2.2(g) to maintain a summary record of student immunizations on form A-45 form.
The links below provide useful information from the NJ Department of Health's Vaccine Preventable Disease Program on childhood and adolescent immunization scheduling, and have been updated to include COVID immunization information.
Please contact your child's pediatrician if you have any questions pertaining to the NJDOH vaccine regulations. Your child will not be able to attend school without the proper scheduled immunizations, as it is the state law.
