Sports Physical Information

Sports physicals must be completed by a primary physician and must be submitted to the Nurse's Office by the assigned due date for each athletic program season for a student to be eligible for participation in the following sports programs:

Volleyball (Grades 6-8) - September 30, 2025

Traveling Basketball (Grades 6-8) - December 11, 2025

Field Hockey (Grades 3-8) - March 19, 2026

Softball (Grades 6-8) - March 19, 2026

Please see our Sports Physical Packet for a full list of meetings for practices and games, as well as the pre-participation form provided to a primary physician for completion. 

The Sports Physical packet is valid for one (1) year.
