NJ FAMILY CARE
Students and families who are in need of health insurance are encouraged to contact
NJ Family Care @ 1-800-701-0710 or www.njfamilycare.org
The Department of Human Services is mailing easy-to-complete applications for NJ FamilyCare to parents who indicated on their 2009 NJ 1040 tax return that they had an uninsured child in their home. As you know, NJ FamilyCare is free or low-cost health insurance for uninsured children and some low-income parents. Children with health coverage are more likely to get the preventative care they need to stay healthy and are ready to learn.

Parents should look for a yellow envelope marked, NJ FAMILYCARE EXPRESS LANE APPLICATION FOR HEALTH INSURANCE ENCLOSED, complete the application, and return it to NJ FamilyCare.
Applications are printed in both English and Spanish. The state also has a translation service and materials available in 13 additional languages. Information provided to NJ Family Care is used only to enroll children. For help, call 1-800-701-0710, or visit www.njfamilycare.org.
Sports Physicals
NJ State Law requires that all students
particpating in extracurricular sports activities have an Athletic
Pre-Participation Physical Examination Form completed by the school
physician or their private physician. Sports physicals can be completed
for free by the school physician. Please contact the school nurse,
Gayle Carroll at 856.451.6610 ext. 106. Sports physical forms can
be downloaded from the links below. Adobe Acrobat Reader is required
to view the downloads. For more information on sports physicals,
